Can ACL Injuries Affect Men and Women Differently?

Studies have shown that female athletes in sports requiring jumping and pivoting are 2-10 times more likely to suffer from an anterior cruciate ligament (ACL) injury in comparison with their male counterparts. Scientists have identified two main reasons for the increased risk, namely, decreased neuromuscular control at the trunk and increased participation by females in athletics.

Basically, an ACL injury refers to the tearing of the anterior cruciate ligament located inside the knee joint. It is most common in sports like basketball, soccer, and volleyball where sudden stops in movement, changes in direction, and hyperextension of the knee are common. It can also occur during an awkward landing from a jump and from falls, as is the case in downhill skiing where women are also becoming more active.

The signs and symptoms of an ACL injury are almost immediately apparent, not to mention they require appropriate medical attention to prevent complications and hasten healing. These are similar in both men and women but the severity of the injury will differ because of the differences in physical anatomy and neuromuscular strength between the two sexes.

Signs and symptoms of an ACL injury include:

  • Loud popping sound
  • Severe pain in the knee followed by swelling and the inability to continue with activity
  • Feeling of instability especially when weight is placed on the joint (i.e., standing)

Doctors assert that the higher risk for ACL injury among female athletes can be attributed to high lateral trunk motion and knee valgus torques. In women, most of the body’s weight is shifted on the injured knee with the foot placed in a lateral position in relation to the body’s center of mass. The increased risk also comes from the deficits in neuromuscular control in the lower extremity, which then increases the strain on the ACL.
